Resolved Companion Death Rate near 100%

Yes, I used mods.
Since the last major patch and the implementation of the death system, I have experienced a near 100% death rate for companions who were knocked out in battle (did not say died per the in-battle scoreboard). There were even battles where I purposefully left companions in their own custom group in the battle and placed them away from the fighting. I confirmed they did not fight, had no kills, and certainly were not knocked out or dead. After I finished those battles and went to the world map, as many as 3 companions had died during the battle. This death rate included companions created through the Distinguished Service Mod and the Vanilla companions.

Hello, unfortunately, we can not offer support for the modded games. Please let us know if you can reproduce the issue on a new campaign after manually removing the mod files and verifying the integrity of the game files.
