Resolved Rescue Your Family quest bugged, no Hideout spawned

Summary: Accepted the quest from Radagos to rescue your family and attack the hideout. I started it in Ocs Hall, however no Hideout spawned after accepting the quest. It's been a few in game years since then and it has not spawned anywhere. I have verified my game files already and it has not fixed it, I also uploaded the save file that this is occurring on.

Do you have a save file before the quest is taken? Also could you try to reproduce this on a new/fresh campaign?

I do not have a prior save file prior to that quest on this campaign. I started a new campaign and was able to get Radagos to give me the quest from Ocs Hall, and the hideout appeared properly this time. I checked that location on my bugged save file and there is no hideout at that location.
