How to Reproduce: Start a new game, and select ironman mode, you'll get normal mode instead.

Same issue for me, before I was playing sandbox ironman with mods, and it worked, later on tried playing campaign without any mods and ironman and it just wasn't working (like above I was getting normal mode instead), and right now even sandbox ironman doesn't work. Must be the 1.2.2 patch that causes the issue, as in 1.2.1 there wasn't such issue

I've got this as well. First game I thought I just forgot to check it. Nope. Check it and doesn't register in game.
