Are there dialogs that should increase relation but don't?

"I heard that you captured Count x. Such chivalry does credit to our cause" sounds like a dialog that would increase your relation with the lord; nevertheless it doesn't. I think this dialog should add a bit relation with the Lord who says it.

I've stumbled upon other similar situations but I don't remember those dialogs. I will post when I find them.

Maybe it should, but I think it is there to give insight to the personality of the Lord for future use.

You are getting faction with those that say that has you gain honor already anyway, so don't see why they should add an other extra relation. =)

I'm not sure but I think it only increase your relation if you have less than 5 or something in relation with this lord since I remember getting relation points for that.
