He says that there are four different possibilities for the "surprise option":

I have, however, only identified three:

1. Brown Noble Mail
2. King Jute Helmet
3. Nobleman sword

I cheated in order to buy over a hundred of the items, but never encountered a four option.  Is this a bug? Or a typo?

I did the exact same thing as you and encountered the same problem. This was on 1.38 though and I haven't yet played 1.39 (damn university work!). Which are you playing? I know that 1.39 is meant to introduce some new special weapons/armour.
