You must have 18 Intelligence and 18 Charisma, and the Infinity Stone, and then enter the Underworld Gate in Veidar. Make sure you have room in your inventory or the Lich's Crown will overwrite the first item.

I have a level 20 ghost with exactly 18 intelligence and charisma, and this isn't working for me. I do have the Infinity Stone and I've done a version check, but when I enter the underworld Cerberus murders me in the face.
