Not sure how long this has been in the game, but besieging a castle and then trying to starve it doesn't work. Eventually the castle will run out of food (30 days) and the castle screen will say they are starving, but nothing changes from that point on. I've starved a castle for 60 days (that's 30 days until they ran out of food, and then 60 days more after that) but they never surrendered, nor lost any manpower.
Edit: I also urged the castle commander to surrender every one of those days (that's 90 days total). And this is for a castle, not a city.
