Does it really mean anything if a town likes you? Is there a discount on items?

Yes there is a discount which scales with the amount they like you.  And in villages you can recruit more and better quality men.

Definitely knew about the village perk. I remember one time in the original MB I went to a village that I spent many days helping with quest after quest and had 87 support and I went to recruit some men and got Men at Arms.
