Crouching. It seems like a basic, staple function in any kind of third/first person action game. You may ask, "how could crouching help in a medieval sword fighting game?" Well, on the hand-to-hand front, it would add an entire new dimension to combat. Being able to duck under sweeping blows would change a lot. Of course that's a lot harder to do with a hundred pounds of armor on, so there would have to be something to count for that, but for the non-tanks among us it would be a game changer.
There's the obvious using cover on top of castles or behind stuff on the ground to avoid archer fire.
then, this is the biggest thing in my opinion, mods. Taleworlds made this game awesomely mod friendly, and they had to have guessed there would be gun mods. A shootout without a way to effectively use cover is none too realistic. Like WWII china battlefield, my favorite mod for original m&b, would be so much better if you had a better way to use cover. I remember williamberne making it so you could go prone, but you couldn't shoot or reload from there so it was pretty pointless.
