Seemingly 'impossible' or 'very hard' quest help!?

It's probably just me, but I have trouble completing the following quests:

Break vassal's family members out of prison: Done it twice, both times I can't actually find a 'key' even after killing/bribing whatever the prison guard... It doesn't automatically unlock the bars or anything either, so I just sit their fighting away or hiding out in the prison unable to rescue said person, I also can't find out how to ransom them, do I need to have good relationships with his captors?

Follow The Spy: I never see the spy leave the city, literally, never.

I'm sure there is another one but can't recall it... :/

Anyway help of any sort is greatly appreciated as this is really bugging me now!

Follow The Spy: I never see the spy leave the city, literally, never.

Whether you see him or not depends on the camera angle so he is easy to miss if you are unlucky.
This quest is easier when you have a skilled tracker with you - just follow the tracks made by a single unit.

Without a skilled tracker I would advise not to accept the quest as it's difficult to follow the spy and stay out of his sight without one.
