I agree that it's not historically plausible to have a player creatable Kingdoms, i.e. "Kingdom of Smurfs of Greater Mazowia" in the game.
Nevertheless, what if the player could consult a NPC 'historian' and lay claim to a historical kingdom or republic in Eastern Europe?
For instance, a player could lay claim and with a mercenary army (and a captured castle or city in the correct region) could re-create the following states:
Prussia (Kingdom)
Finland (Kingdom)
Livonian Confederation (Prussian Elite and Latvian + Estonian Republic)
Kurland (Latvian + Estonian Monarchy)
Novgorod (North West Russian Republic)
Kiev Rus (Ukrainian Kingdom; as opposed to the Republican Cossacks)
Principality of Polotsk (Belarus Kingdom)
Lithuania
If a player does re-create a kingdom than he can title himself a new Dynasty (and with a dynasty we will need a heir... hence the need for a wife mod

Nevertheless, this would be the ideal option for players who want to become a leader of their own faction and not deal with NPC Pretenders.
Once a player creates his kingdom or state, then he would be approached by descendents of disinherited nobles / plutocrats of those respective regions and these descendents would have realistic names. I.E., your Finnish kingdom would have Finnish nobles with Finnish names. Or if you have recreated the Livonian or Novgorodian Republic than you would have "ministers" and "generals" with Baltic and Russian surnames.
