Resolved Quest Failed and Battle Death Glitch

I loaded into an existing save in 1.5.5 Previously, I had been putting off the quest where I can choose to give the assembled dragon banner to either Istiana or Arzagos. Pretty quickly after I loaded the save the quest failed saying I had not chosen either character within ten years. In the quest log it says that the mission was given to me within 200 days so obviously that is not the case.

Also, I had been playing with battle deaths disabled. After playing for some time I noticed that characters began dying from battle. I tried to re-disable battle deaths but found that where the option would normally be under "campaign options" it could not be found at all. I noticed that "enable tutorials" and de-selected it but I doubt that has anything to do with it.
