No, I didn't use any mods.
I arrived to this post because I am having the same issues and I was looking for some information. However, in my case it happens during the battles. You run around and when you move the mouse right and left the lag is just there. Sometimes aiming with the bow becomes a challenge.

Actually, I did not have any issues previously. I equip a GeForce GTX 1660 and 8 gb.

I have not played this game since around 1.4.1, then it ran smooth. The latest build has a bunch of pauses between different screens (character, inventory, party), about 3-5 seconds, I can move my mouse around waiting like it is a 90s internet porn page. Then on the map there is a 1 second pause as my character moves about every 3 steps. This game has gotten progressively worse in performance, I have an RTX 2080 super, anyone have any settings for a less choppy performance?

While waiting I decided to uninstall the game, remove the main folder and reinstall. It seems to work now, I still have the file if you are interested in it and have a way to receive it.
