I don't know how it works now but I think you have to be in a settlementto get pregnant.
My main hero has 1 kid while my other clan members have an army [edit] even with them teleporting everywhere to participate in tournaments

Maybe Bannerlord will have Dickplomacy made for it, and creating children can therefore be a manual action, as opposed to a passive one.
