How often does "crush_through" (or extra penetration) proc/activate?

I think so. Maybe they even backported it into Warband, I haven't really checked for changes in the new versions.

There are many things in the code that don't really make sense: leftovers, oversights, paths that are never taken... standard programmer stuff.

I think you forget that mounted thrusts can still crush through (although much more unlikely thanks to the line of code above).

Does it means that ground thrust can still have the same effect (even if unlikely  ? )

Even I understood, that yes, it can. But only if you are mounted, and they will have halved crush factor.
