Right now, when you join the army all you get is a pauper weekly wage and a few quests, most of which aren't worth their reward (i.e. raising a couple of knights and handing them over).
What I'm suggesting is to add a new npc in the Count's castle (or another building in cities, called "Barracks"). This npc would only talk to you if you're in his army, and he would have better tier units for hiring (instead of just peasants and 3-5 militias/footmen).
The quality of the units would depend on your rank in the army (so, no knights for regular soldiers). They'd have a high joining fee, and wouldn't be too numerous...
Most of them should be swadian footmen/vaegir veterans, with a few swadian man at arms/vaegir horsemen if you have a high rank. Maybe a knight or two if you have a very high rank..
We all know how hard it is to keep your army alive. Raising a band of 40 decent soldiers can take a very long time, but war parties of 50+ enemies spawn every now and then, fully formed; you can't afford to lose half your men in every important battle.
The surgery skill is very unneffective unless you have a very high level in it; but if you do, you will be lacking in combat skills.. Marnid and Borcha level up way too slowly to be useful in this way..
