Need More Info Graphic issue for months

Since official release, I crash everytime I access character generation page or load a previously saved game (with mods). This is a D3D bug I'm unable to locate.
I made a clear install of the game, uninstalled/reinstalled the graphic driver, nothing changed.
Before the clean install, I sent you bug reports, but after the clean install, I don't have access to bug reports, game freezes and I have to switch off the PC.

I played 693hrs during Beta and now the game is officially released, I can't play it!

Were you able to see the crash uploader tool? If you can upload your crash to us we can check it out. Please don't forget to copy the Crash ID and write it here so that we can find it.

Crash uploads were sent before I made the clean install and after it, computer freezes everytime.
Before the clean install, I could see a pop up mentioning D3D issues but I couldn't make screenshots.

The message of error is D3D_device_context_->Map at rglGPU_device::lock_texture Failed.
It happened with the V1.0.2.8368.
My NVidia GPU driver is up to date.
