Yeah. A sexy tavern wench who, if a player so much as talks to her, pulls out a rolling pin and beats him senseless. That'd be hilarious. (And make sure you take it out before release...we've already got one Bard's Tale in the world).
Aside from that - is ambient music possible? Ambient sound? Crackling fire, minstrel tunes, knucklebones rolling, mugs clacking, folks talking and laughing.
Also more people. If this really is like a central fortified inn you'd probably have as many stacks of loose tradegoods around as decorations. It's a medieval truckstop! So wagons, barrels, wheels, stables, shacks and workshops of all kinds. Storehouses. Wooden pallisade probably. In fact most construction should probably be wooden and makeshift, if worn, well-used and comfortable looking. It'd differentiate itself better from stonebuilt cities. Illustrated tapestries, patterned rugs, various souveniers on the walls possibly taken from bandits or as trophies of resisted attacks from Khergits and nomad raiders. Maybe some hunting trophies - deer heads and the like. Rushes strewn on the floor in common areas (sweeping 'em out is easier than cleaning the rugs!). Cooking impliments on display (and probably not very clean looking) along with food and drink.
Miscellaneous possibilities: tollhouse, tent-maker, mill, brewery, well, fletcher/smith. And a private bathroom you need a key for but you have to buy something first. I kid, I kid.
