Fief health

Playing latest 4.1 release, all of my fiefs are at the highest wealth level (and have been for quite some time).
I always click the most positive (and usually expensive) option for dealing with any and all issues that arise.
Despite all this, my fiefs are all at "poor" health or worse.

Does health have any affect I should be worried about?
Am I doing something wrong?
Is there some way to actively promote good health? (If not then there should be - some kind of "Barber-surgeon" building or whatever is appropriate to the setting)
 
I just put in code today that actually makes health change over time.  Prior to this, all versions of SoD just had health as a static value that only changed if you happened to get a king-event that let you respond to help or hurt it.

So, it should move towards average health now, with an occasional event to send it plummeting or rising, plus the events from before.

As to all of your fiefs being 100% prosperity - that sounds extremely unlikely to me.  Villages generally max out at around 80 even under ideal circumstances.  Towns go to 100 (or above) easily enough, however.

Anytime you get raided, your villages plummet, and it takes time for them to build back up (about 5% per week).
