The PrecisionX kinda gives it away. Drivers are looking for specific results from the hardware polling, if you've got anything that's going to return unexpected results, the driver will fail because it can no longer identify the GPU.

No, that does not neccesarily imply that he OCed the card. By the way, EVGA GTX 560 Ti Superclocked has 900MHz core and 4212MHz memory by default, and I'm 99,9% sure that he has this card.
 
If your overclocking a gpu and it fails it will most likely revert back to stock. It won't report a higher clock speed from overclock if it isn't stable.
 
It will report that clock until it crashes. For example my 460M will run at 800MH/z up until around 10 seconds of load, upon which it crashes and reboots itself.

This is presuming he's not overclocking in the BIOS.
 
you have to disable card power saving mode from nvidia control panel and enable best performance instead.
i used to have the same problem but this fixed it, i got the gigabyte gtx 560ti OC version though.
 
Has no one suggested turning off PrecisionX's framerate/clock target? It could adversely effect the nVidia drivers.
