I don't very often, and did at first because I wanted to see what it did. After I did though I called a meeting with the castle commander and they surrendered On the other side though, I think that mightve been because I had a lot more troops than they did. Or maybe because they didn't have any supplies anymore. I am really not sure. But that's what it does, cuts off supplies no matter what.
I am too used to Warband's "build ladder in 5 hours" to siege, so I never have the patience Also if I do this, in the meantime the enemy could take up to 2 of my own castles without me knowing.
I use it. Especially when sieging towns, where I divide my forces up in two armies due to low renown --> low party limit. The first army is supposed to make the enemy garrison dwindle but if the enemy has 500-700 and I've got ~350 in each army the random events will end up with about 100 of my guys dying every day. Building the circunvallation doesn't mean you lose anything. The siege might end up taking longer than you expect.
Sides, you can build several things at the same time, which means you don't lose the four days.
I also like to use the spy and stuff. Most of my sieges last more than four days all in all.
From a strategy guide in the tutorial section, it seems you can build Circumvallation and Drainage Ditches (or whatever the sewage thing is that keeps your men dying of dyssentry) at the same time.
Last time I tried I lost more men during siege building than when I tried a direct assault.
Not to mention the nearly the impossible morale management. ( they should add a whore in the siege camp xD)

That perhaps suggests a balancing issue. I like the idea of more complex, drawn out, realistic sieges, but they have to make sense in terms of the greater gameplay experience. Ofcourse difficulty settings probably have a hand in that.
To build two things at the same time simply click on the ground immediately after starting to build something and then pick something else to build from the menu.
The extra defence options? I get the AI thing but why not the defence options? I'm saying because the enemy always seems to rape your guys when they ninja them while all you can do is wait in your town waiting to be attacked.
That said, I know that in the medieval period atleast, wooden structures were sometimes built on top of the walls to give defenders an advantage, and to make placing ladders harder. I think probably most work by defenders would be repairs though, and perhaps not even that.
You could potentially add an option, as a defender, to attempt to sally out and get supplies in. The reverse of that special battle could be a random event when players besiege cities.
A lot of the problems come from the way sieges behave for players vs how they behave for AI. It'd probably be a fair bit of work to simulate the same effects on the AI that affect players however.
