Closed Display gets disconnected randomly and need to restart PC to fix it

Summary:while playing randomly the display gets disconnected and i need to restart the pc to fix it

Have you used cheats and if so which:gave my character level 300 in charm,smithing,medicine.As for the mods this happens with vanilla , the old realms mod , and realms forgotten
