Disappearing shield

I've got a cracked steel shield but after using the inventory during a battle it disappears! Hes still holding the sword and can swap to a bow but the shield never comes back.

Am I doing something wrong? (Version 0.621)

You should reequip it by pressing [ button.

I only had the one shield so I thought if I swapped back to sword he'd use the shield he was holding automatically. Could I request that for the next version?

One other thing, maybe this one is more of a bug, I don't know if anyone else has had this happen -

I started half way up a cliff, the inventory chest is there too, as soon as I moved the horse fell down the cliff and died
