When you win a siege and choose the option that requires influence like mercy or devastate the town there is a chance some companion (not family) members who are running a party die or maybe leave you for doing it, only thing I can thing is they are dying or leaving for low loyalty. I am including a save file called deaths.sav and also this quick video so you can see and replicate it yourself.
***UPDATE*** we think when relations drops below -10 they would normally talk to us and we could decide how to answer, but it just seems to dismiss them, so it could be if any companion gets under -10 they will just leave, that has not been tested yet

I had this happen on 1.8 The Companion party in question was run by someone with the "cruel" trait, and when I did show mercy and took the fief, he took a standing penality and left... afaik the penality was -15

If they leave if your standing drop low enough, I won't call that a bug though.

I have had three companion clanparties disbanded in 5 minutes of each other. With also meant loosing the clanmembers I have spend ages to build and millions of gear lost 😞

For the issue in regard of OP, never have an army with a clan party that is lead by a companion with cruel traits if you got mercy. Try have your clan parties lead by someone with same traits as your own, or neither, as they would be indifferent to what you chose in that case.

Thanx for your answer. But after I lost a previous clanmember I checked the stats and changed them for characters with the mercy trait.
Just to be safe I choose the most neutral option after the siege, thinking it would not have an effect, but I think I should have stuck with the mercy option.
 
I've been informed that this bug is fixed and the fix will be sent to the game with future patches. This is the explanation I've been given:
Companions that leave the clan or are kicked from the clan get lost. (he/she doesn't die, they get lost) Here, after the siege, the player chooses to devastate/mercy option which will decrease their relation with some of the companions. Due to relation loss, they leave the clan so they get lost.
