i understand some of it might break but everything gone seems a little harsh or is there a drop chance?
how does this work

Its just gone along with the companion. Its kind of sad that they die without even a notification to remember them by. I think it says they died in the feed but that flies by so fast that its real easy to miss and you might not even notice theyre gone if you don't look closely at the after battle report.

The mod Distinguished Service used to have its own death mechanic before death was activated in the main game. It was nice because when a companion died, you got a big popup telling you so, and you had a choice of sending their gear back to their family for a morale boost or you could keep it and take a morale loss. It lessened the blow of losing your companion a little when you could at least get back the Noble Longbow you just bought for him right before the battle

If you're on 1.5.3 there's a bad workaround, lords don't actually get removed from the game until the whole battle is over, so if your companion/family goes down you can retreat and open inventory and take their gear off, then attack again.....
Of course you could just re-load the game as easily and not lose them so it not much or a workaround.

Something like this should be in native BL.
