Ranged participants switch from ranged to melee too late, they always eat the first strike and they lose automatically thereafter. Maybe they don't calculate the potential running perks and high athletics values of enemies that are frequent in the arena, but it's just a guess.
It appears in every single arena fight with ranged weapons, no special circumstances are required.
It might be okay on the battlefield, but it's very obvious in the arena.

After some play, it appeared once between billhook fighters when both of them missed their first attack.
 
No, I don't make videos, I don't have software to record and edit.
It happens all the time. Vlandia and Batania have many arena fights with bows and crossbows included. I observed my companions with very high weapon skills (130+) sometimes are able to draw their melee weapons in time. But it might be a coincidence as well.
